You have learned, in the preceding lessons, how to confirm enrollment eligibility, execute a randomization transaction, and document the enrollment event across five parallel systems. These are the mechanics of enrollment β the operational steps that turn a screened candidate into a study participant.
But there is a dimension of enrollment management that extends beyond the individual participant, and it is one that the sponsor cares about deeply: Are you enrolling on schedule? How many participants have you enrolled relative to the target? What does your pipeline look like β how many candidates are in pre-screening, how many are in screening, how many are likely to reach enrollment in the next two weeks? And if you are behind target, what are you doing about it?
These are not casual inquiries. The sponsor has built a study timeline around enrollment projections. Drug supply has been manufactured and shipped based on anticipated enrollment rates. Statistical analyses require a minimum sample size. Regulatory submissions have target dates that depend on trial completion, which depends on enrollment. When a site falls behind its enrollment target, the ripple effects extend through the entire study β delayed database lock, delayed regulatory submission, delayed patient access to a potentially effective treatment.
And yet β and this is the tension that makes this lesson essential β the pressure to meet enrollment targets must never compromise the integrity of the enrollment process. Not once. Not for one participant. The moment a coordinator enrolls a participant who does not fully meet the criteria because the site is behind target, the entire regulatory and ethical framework that protects research participants has been violated. The pressure is real. The boundary is absolute.
