Every clinical trial protocol is a carefully designed blueprint. It specifies visit windows, procedure sequences, dosing schedules, laboratory parameters, and eligibility criteria with a precision that leaves little room for ambiguity. And yet, in the real world of clinical research β where participants arrive late, pharmacies delay dispensing, blood draws are missed, and consent forms expire between versions β the protocol is not always followed exactly as written.
When the protocol is not followed, a deviation has occurred. And the coordinator's job is not to pretend it did not happen. The job is to recognize the deviation, understand its severity, classify it appropriately, and route it through the correct reporting pathway. This is not about punishment. This is about transparency, participant protection, and data integrity β the three values that the entire regulatory framework exists to safeguard.
I have reviewed hundreds of deviation logs across dozens of sites over the years, and I can tell you that the sites with the best quality records are not the ones with the fewest deviations. They are the ones that classify and report deviations honestly, analyze them thoughtfully, and use them as the raw material for improvement. The sites that worry me are the ones with suspiciously clean records β because no site that enrolls participants in clinical trials has zero deviations. Zero reported deviations does not mean zero deviations. It means zero recognized or reported deviations, and that is a much more concerning signal.
