The question corrective action does not ask

The previous lesson taught you to design corrective actions that actually address root causes -- process changes with scope matched to the finding, specificity sufficient for implementation, accountability assigned to an individual with authority, and verifiable completion criteria. If you designed those well, the system that produced the finding has been changed so the specific failure cannot recur through the same causal pathway.

But here is the question corrective action does not ask: where else might that same design weakness exist?

This is the intellectual leap that separates corrective from preventive action, and it is -- in my view -- the most underappreciated competency in site-level quality management. Corrective action looks backward: a finding occurred, the root cause was identified, and the process was changed. Preventive action looks sideways: the root cause revealed a category of systemic vulnerability, and the RC now examines other processes for the same category of weakness before those processes produce their own findings.

Consider what happens when a site's root cause analysis reveals that inconsistent IRB continuing review timelines across the portfolio stem from the absence of automated deadline tracking. The corrective action creates that tracking system for IRB continuing reviews. Done. But the RC who stops there has addressed one process. The RC who asks "what other regulatory processes at this site depend on manual deadline tracking with no automated failsafe?" has identified a vulnerability class -- and every process in that class carries the same design weakness that produced the original finding. Safety reporting timelines. Annual investigator brochure updates. Protocol deviation reporting windows. Financial disclosure renewals. Each of these processes may be operating right now with the identical gap that just produced a finding in the IRB domain.

Preventive action addresses the class, not just the instance.
