Teaches RCs to design consent form version control systems that track the relationship between protocol versions, consent form versions, and IRB approval dates -- and to build detection controls that catch version mismatches before they reach participants.
A monitor arrives for a routine visit and pulls the most recently signed consent forms from the regulatory binder. The consent form on top reads "Version 3.0, dated September 14, 2025." The monitor checks the IRB approval letter. The approval letter, dated October 2, confirms that consent form version 4.0 was approved for use -- two weeks before the last three participants signed version 3.0.
Three participants consented on a superseded form. The version 4.0 consent included a new section describing hepatotoxicity signals identified in a concurrent trial -- information that Section 2.8.2 of ICH E6(R3) requires be communicated to participants "in a timely manner" because it "may be relevant to the participant's willingness to continue trial participation." Those three participants made their continuation decision without it.
The investigator's regulatory binder had the approval letter filed correctly. The IRB portal showed version 4.0 as the active consent. Every upstream system worked. But the clinical team was still handing participants version 3.0 at the consent table because no one told them to stop. The version control system failed at the last step -- deployment to the point of use.
This is not a hypothetical. I have seen this exact failure pattern at academic medical centers, dedicated research organizations, and community sites. It happens not because people are careless but because the version control system was never designed with enough redundancy. And it is entirely preventable -- if the RC builds the right infrastructure.
