I want to start with an observation that does not appear in any regulatory guideline but shapes every multi-IRB portfolio I have encountered: the most insidious cost of managing multiple IRBs is not the additional submissions, the different forms, or the varied timelines. It is the cognitive cost of context-switching.

When a CRC or RC moves from preparing a WCG submission to preparing an institutional IRB submission to checking the status of a cooperative group CIRB application, each transition requires a mental gear change. Different portals. Different terminology. Different form fields. Different assumptions about what documents are required, how they should be formatted, and where they should be uploaded. This constant switching produces errors -- not because the coordinator is incompetent, but because the human brain is not optimized for maintaining multiple parallel procedural frameworks with superficially similar but operationally different requirements.

I have watched experienced coordinators submit consent forms to the wrong IRB portal because both were open on adjacent browser tabs. I have seen amendment forms rejected because the coordinator used the continuing review template by mistake -- the forms looked similar enough that the error was not caught until the IRB returned it. These are not knowledge failures. They are systems failures -- and they call for a systems solution.
