Build tracking systems, reminder workflows, and quality checks that ensure no safety obligation falls through the cracks at your site.
Consider this scenario. You are managing three active studies with a combined enrollment of 47 participants. Across those studies, you have 23 open adverse events in various stages of documentation, four pending SAE follow-up reports with sponsor-specified deadlines, 12 unacknowledged IND safety reports that arrived over the past two weeks, and an Investigator's Brochure update that landed in your inbox three days ago. A monitor calls to schedule a visit for next Tuesday.
Can you answer these questions right now, without searching through email threads or flipping through binders?
Are all adverse events for every active participant fully documented -- source document, CRF, severity grade, seriousness assessment, causality determination? Have all SAE follow-up reports been submitted on time, or are any overdue? Have all incoming safety communications been reviewed by the investigator, acknowledged to the sponsor, reported to the IRB where required, and filed in the regulatory binder? Has the IB update been processed -- investigator notified, new safety information reviewed against current participants, acknowledgment returned?
If you cannot answer those questions with confidence, you do not have a systems problem. You have a visibility problem. And in safety reporting, what you cannot see is what you will miss.
