Understand the DSMB's role in participant protection, process its four recommendation types with appropriate site-level actions, and maintain blinding integrity throughout.
A one-page letter arrives from the sponsor on a Wednesday afternoon. The header reads "DSMB Communication -- Study XYZ-401." The body is three sentences: "The Data Safety Monitoring Board convened on 22 February 2026 for its regularly scheduled interim analysis. After reviewing unblinded safety and efficacy data, the DSMB recommends that the study continue without modification. No action is required at the site level."
Three sentences. No drama. The study proceeds as planned.
But consider a different letter -- same header, same board, same study. This one reads: "After reviewing unblinded safety and efficacy data, the DSMB recommends immediate suspension of enrollment pending further analysis of a safety signal identified during the interim review. Sites should cease screening and enrollment activities effective upon receipt of this communication. Currently enrolled participants should continue per protocol pending further guidance."
Same committee. Same study. Entirely different Wednesday afternoon.
The Data Safety Monitoring Board is the only body in a clinical trial that sees all of the data -- unblinded, aggregated, analyzed -- and has the authority to recommend that the study change course. When the DSMB speaks, sites must listen. And the coordinator must know what to do with each type of recommendation, because some require immediate operational changes while others require nothing more than documentation and filing. The difference between these responses is not a matter of preference. It is a matter of participant safety.
