FDA Form 1572 is a single page. It fits on one sheet of paper. There are no appendices, no supplementary modules, no multi-section exhibits requiring cross-referencing. And yet this single page carries more legal weight than any other document in the investigator's regulatory file--more than the protocol, more than the clinical trial agreement, more than the informed consent form.

When an investigator signs Form 1572, that signature is not an acknowledgment. It is not an administrative checkbox. It is a personal, legally binding commitment to the United States federal government that the investigator will comply with a specific set of regulations governing the conduct of a clinical trial under an Investigational New Drug (IND) application. Violating those commitments can result in warning letters, clinical holds on the study, disqualification from conducting future research, and--in the most serious cases--criminal prosecution.

I have watched experienced investigators sign this form without reading it. I have seen coordinators prepare it like any other regulatory document--printed, placed in the signature pile, filed after signing. And I have observed the consequences when the commitments on that page are not understood by the person whose signature appears at the bottom, or by the team responsible for helping fulfill those commitments day after day.
