In the previous lesson, you learned to think in terms of Critical to Quality factors--the specific processes where failure threatens participant safety or data reliability. You built a mental map of where errors carry the highest consequence: eligibility verification, consent integrity, data accuracy, visit window compliance. And you practiced embedding verification triggers at natural transition points in your workflow.
That lesson answered the question: what should I protect most carefully?
This lesson answers a different question: how do deviations actually happen, and what can I do to stop them before they start?
I want to be honest about something. In over three decades of teaching and consulting at research sites, I have never met a coordinator who caused a protocol deviation on purpose. Not once. Deviations do not happen because coordinators are careless or indifferent. They happen because protocols are complex, workdays are long, human attention is finite, and the conditions under which coordinators work--multiple concurrent studies, constant interruptions, competing priorities--are precisely the conditions that cognitive science tells us produce the most errors. The coordinator who believes "I would never let that happen" is, paradoxically, the one at greatest risk. Confidence is not a prevention strategy.
Prevention is a prevention strategy. And that is what this lesson teaches: a systematic approach to identifying where deviations cluster, building verification tools that catch errors in the moment, recognizing the human factors that degrade your performance, and understanding how deviations are classified so you appreciate what is truly at stake when prevention fails.
