The participant wants to sign. You are not sure they should.

After three teach-back cycles, the participant still cannot explain what randomization means for their care. Each time, the coordinator tried a different approach -- an analogy, a visual, simpler language. Each time, the participant nodded along. And each time, when asked to explain the concept back, the participant returned to the same answer: "My doctor will decide which treatment is best for me."

The participant is not hostile. They are not disengaged. They are eager -- genuinely eager -- to join the study. They have traveled 90 minutes to get to the appointment. They brought a folder of medical records. They have told the coordinator, twice, that they are "ready to sign."

And the coordinator is facing one of the hardest decisions in the consent process: the participant wants to enroll, but the coordinator is not confident that the participant understands what they are enrolling in.

This is the lesson the previous three have been building toward. Lesson 1 established why passive comprehension checks fail. Lesson 2 gave you the teach-back method. Lesson 3 aimed that method at the four concepts most commonly misunderstood. This lesson addresses the question that arises when all of those tools have been applied and the result is still inadequate: what do you do now?

The answer is not always obvious. And it is almost never comfortable.
