If IP receipt and storage represent the first links in the accountability chain, dispensing is where the chain connects to the participant. And it is the link where errors carry the most immediate consequences. A storage temperature excursion might compromise product stability over time. A dispensing error — wrong participant, wrong kit, wrong dose, wrong visit — can harm a participant today.

I want to be direct about something that I have seen play out at multiple sites across my career: dispensing errors almost never result from carelessness. They result from shortcuts. The coordinator who "knows" which kit goes to which participant and skips the verification. The coordinator who dispenses from memory during a busy clinic day instead of checking the randomization system. The coordinator who hands a participant their medication and says "same as last time" instead of providing complete instructions. These shortcuts feel efficient in the moment. They are catastrophic when they produce an error.
