Maintain the IP accountability log, perform reconciliation at defined intervals, prepare for monitoring visits, and manage IP return and destruction procedures.
There is a fundamental equation in investigational product accountability, and it is one of the few equations in clinical research that admits no rounding, no estimation, and no approximation. It is this:
Units received minus units dispensed minus units returned minus units destroyed equals current inventory.
Every unit must appear on one side of that equation. A unit that was received but cannot be accounted for — not in inventory, not dispensed to a participant, not returned to the sponsor, not documented as destroyed — is a missing unit. And a missing unit is not a data management problem. It is a regulatory finding that triggers questions about diversion, about accountability, about whether the site can be trusted with investigational product.
I have seen inspectors walk into a pharmacy, count the units on the shelf, compare the count to the accountability log, and know within 60 seconds whether the site's IP management is trustworthy. The math either works or it does not. And when it does not, the questions that follow consume days.
