The visit begins before the participant arrives

There is a moment, perhaps 30 minutes before a participant arrives for a study visit, when the coordinator scans the exam room and realizes the lab kit for this visit type is not on the shelf. The pharmacy has not been notified that the participant needs investigational product dispensed today. The ECG machine's calibration sticker shows it expired last week. And the participant's chart reveals an adverse event from the last visit that the investigator has not yet reviewed.

This is not a rare scenario. It is what happens when preparation is treated as something you do in the 15 minutes before the participant walks in, rather than as the structured, protocol-driven process it must be.

Visit preparation is, in my view, the most underappreciated competency in clinical research coordination. It receives almost no attention in training programs, yet it determines whether the visit itself runs smoothly or becomes a series of improvised workarounds. The coordinator who has prepared — who has confirmed the lab kit, verified the equipment, coordinated with the pharmacy, and reviewed the participant's history — can focus during the visit on what matters: executing procedures correctly, documenting contemporaneously, and attending to the participant. The coordinator who has not prepared spends the visit solving problems that should not exist.
