A monitor arrives for a pre-activation review. The essential records checklist looks strong--IRB approval is in hand, the delegation log is signed, investigator CVs are current. Then the monitor asks for the laboratory certification. The coordinator pulls the CLIA certificate from the file. The monitor glances at the expiration date, pauses, and says: "This expired in December."
It is now March. The certificate has been invalid for three months. Nobody checked.
This is not a hypothetical. I have seen this exact scenario--or a close variation of it--at least a dozen times over my career. And what makes it particularly vexing is that laboratory certifications are among the most straightforward essential records to verify. They have clear expiration dates printed directly on the certificate. They can be verified through public databases in minutes. There is no ambiguity about what "current" means. Yet they are overlooked with remarkable frequency, usually because the coordinator assumes that someone in the laboratory department is handling it, and the laboratory department assumes that the certificate is a research concern, not theirs.
The result is a delayed activation. Not because of a complicated regulatory problem. Not because of a contract dispute or an IRB query. Because nobody looked at a date on a piece of paper.
Laboratory documentation occupies a peculiar position in the essential records collection process. Unlike investigator CVs or delegation logs--documents that the coordinator generates or directly controls--laboratory records originate from an entirely different department, often a different building, sometimes a different organization altogether. The coordinator must collect documents from people who may have no involvement with the clinical trial and no particular motivation to prioritize the request. And the documents themselves are highly technical: accreditation certificates, proficiency testing results, reference ranges with effective dates, laboratory director credentials.
